Project description:

We wish to continue to build on the work we did this year but offer more advice and information sessions in the community and increase the number of free early intervention and prevention surveys. These are carried out during a face to face visit in clients home. All clients are offered a benefit entitlement check and Information and advice is given on various topics, such as Safety & Security within the home, Smart Meters, Minor or major adaptations to the home, Equipment to promote independent living, Dementia Support, access to financial assistance for repairs, improvements adaptations or basic household items, One to one support to progress benefit or charitable applications; accessing services or carrying out repairs improvements or adaptations in the home for those living with a mental health condition and or cognitive impairment, Information Sessions delivered within the Community as well as at Dementia Cafes, Sheltered Housing Sites and to health and social sites.
