Reshaping Care for Older People
The Scottish Government established a four year Change Fund in 2011/12 to enable Health and Social Care Partnerships to develop and implement local plans for making better use of their combined resources for older people’s services.
The Aberdeen Community Planning Partnership has charged the Health and Social Care Partnership (H&SCP) with the responsibility for ensuring appropriate spend of the Change Fund. To discharge this responsibility, the H&SCP established a Partnership group to receive proposals and decide on the award of the Change Fund. For years 3 and 4 of the Change Fund, in total £5.878m, £3.14m for 2013-14 and £2.738m for 2014-15, is available to the Aberdeen Partnership as ‘bridging finance’ to enable the redesign of services or supports that can transform how older people live in the community.
